Download the participant health history form(pdf) and waiver(pdf), fill them out and bring them the on the first day of Bootcamp or Private personal training.

After you have downloaded the forms you can proceed to complete payment online or you can pay by cash perferrable or check in person.

Kutting Weight Policy

1. We reserve the right to cancel a session (usually when fewer than 6 people are signed up for a class). In this case, anyone who has paid will receive a full refund.

2. Class will be held rain or shine.

3. There's NO refund once a payment has been processed. However, a class credit may be given for special reasons (serious medical (require doctor's note) or emergency matters) with proper notice.

4. You must sign the waiver and health history forms and make a full payment prior to starting the session. You will be charged a $20 late payment fee if we don't receive your payment by the start date.

5. All returned checks incur a $25 fee.

6. You will NOT be reimbursed or credited for any missed classes (unless it's for a "special reason" as stated in #3 above). It's your FULL responsibility to make the most out of the 5-week session.

7. Sign-up with friends discount is available only to new customers. Any coupons received through our 3rd party affiliates can be used only by new customers.

8. Only one discount can be used per session. Discounts are taken off the full price.
9. Prices are subject to change at anytime during the year.
10. You need to make every effort to be on time for each class.
11. LATE PAYMENTS: Payments must be made by the start date of the session. Returning clients lose ALL discount privileges if late on payment (e.g. that means paying the regular rate instead of returnee rate). New clients incur a $20 late fee.
